OK so figured it out.. line 1177 of FileUpload.js says:
this.isFlash = swfobject.hasFlashPlayerVersion("9.0.28");
As my flash version was 9.0.124 I would get the empty add button.. once I moved to flash 10, everything was working as expected.
Seems like this is specific to flash/linux as I had tried it on a windows machine with flash 9 and it appeared to degrade gracefully by creating a disabled add button.
