After investigating with Juan we have narrowed down the issue somewhat.

For some reason occasionally when the application is rendered for the first time the a4j:ajax link is not set correctly.

The demo attribute has no value.  All other's links appear fine, and I suspect it has to do with either being the first, or the default selected value.

When it is happening on the GAE version it is fairly consistent.  However it is not always a problem i.e. right now it seems to be fine.  It does not seem to be reproducible on my local machine.
