The following approach should work :
DecisionTableConfiguration dtconf =
KnowledgeBuilderFactory.newDecisionTableConfiguration();
		dtconf.setInputType( DecisionTableInputType.XLS );
		dtconf.setWorksheetName( "Tables" ); // Sheet containing the decision
table

		kbuilder.add(ResourceFactory.newFileResource(
		"<path to file>\\ExamplePolicyPricing.xls"), ResourceType.DTABLE, dtconf);
