tweaked layout to expose button.
The Texttable dialog includes the "inlcude HEADER" option.
Added pre-populating source and view model names with "TextFile_source" and
"TextFile_view where "TextFile" is the name of the selected flat file minus
the file extension

There is a setup of the Format Options configuration in Flat File import wizard.
A HEDER option in TEXTTABLE() function is contained in Generated SQL Statement even if it
disables the 'Column names in header' check box.
