The error most likely means that you did not build all packages in Guvnor before accessing the the process view in the console. Also you say you have generated the forms with the Designer, but the screen shots you attached are  from  https://github.com/droolsjbpm/jbpm/tree/master/jbpm-gwt/jbpm-gwt-form/src/main/resources https://github.com/droolsjbpm/jbpm/tree/master/jbpm-gwt/jbpm-gwt-form/src/main/resources, so you must have uploaded them to Guvnor manually? 
Would be nice if you could step-by-step describe your setup so someone can try to reproduce.
