[keycloak-user] Both the access token and the refresh token have expired

I followed the example given at https://github.com/dylanplecki/KeycloakOwinAuthentication/wiki/ASP.NET-MVC-Tutorial and it works up until the part where you press the login button.

I can navigate to the About page and I am taken to the login page.  I enter the login name and password and press the login button and I get the error "Both the access token and the refresh token have expired".

Does anyone know why this is and what the solution is?
