Added back to application link to account management pages.
It requires the base url to be set, also the base url has to be a prefix of the Referer
header. The application can also use '?referrer=<app name>', if this is set
the Referer header is ignored and the base url for the specified application used
directly.

Yes to both
I think we should only add "back to application" link if Referer matches
baseUrl to one of the applications in the realm.
For links to other apps in realm it would be nice to make the list of
applications available to devs (rest endpoint + support in adapters/sdks) so
they can add something similar to their own applications. It would also be
nice to have a list of applications under the account management pages so
the user can see what applications he can access, maybe including what roles
he has for each app.
