We've seen this exception several times, too.. i guess there is a problem elsewhere,
which causes an increment on a yet nonexistent version value ...
a co-worker of me did found the real cause of this exception in his case:
There is some non nullable value on the object which was not set... so at commit you get
this misleading Exception.
You should get a Not-Null Property exception (or something), but in some cases we get this
NullPointer one.
