I'm re-opening because the code added for caching EntityKey in AbstractLazyInitializer
has a bug. Also, the implementation is a bit too complicated to be very maintainable.
I'll remove the code for caching EntityKey. I'll revisit this if it hurts
performance.

A proxy can be associated with a new session when it is already connected to a different
one.
This happens because AbstractLazyInitializer.isConnectedToSession() has a bug that causes
it to always return false.
