Maybe it will help if you put it to KEYCLOAK_HOME/modules/systems/add-ons/keycloak/org/keycloak/keycloak-server-subsystem/main/server-war/WEB-INF/web.xml to Keycloak Session Management Filter? Feel free to create JIRA for it (and mention if it helps or not. If it helps, then we know that it's sufficient to add it to that filter, so it will be easy work to fix).
