Removed most icons as repetitive and it is now implemented as just a single toolbar icon with sub-menus. You pull down the toolbar icon, there's a list of servers. Each item has its own sub-menu, with actions like start / stop / debug / profile / publish / etc. I've seen no deadlocks at this point.
