Multiple other images and icons also appear. Perhaps something was fixed between the 4.0 and 4.1 docs' release? Is the path to that image relative or absolute? I thought maybe it was an absolute path to a VPN-only URL, but after dropping off VPN other docs referring to that image all loaded fine. If the images are relative paths and they're contained inside a plugin, maybe the problem was the plugin's build.properties failed to include the images when building the binary?
