Steps to reproduce:
1. EXECUTE: Create xhtml file with the following content:

<html xmlns="http://www.w3.org/1999/xhtml"

</html>
2. EXECUTE: Point cursor in one of the following positions: [xmlns:|h="http...],
[xmlns:h=|"http...]
3. EXECUTE: Use CA
4. ASSERT: It works
5. EXECUTE: Delete [xmlns:ui="http://java.sun.com/jsf/facelets"] and save
changes
6. EXECUTE: step 2
7. ASSERT: It works
8. FAILURE: NullPointer exception and String index out of range exception appears
