Also, I mentioned this to Koen Aers last week, and he told me that he did not know about
the 'Use Source tab as a default for multi-tab editors' option, which means that
the jBPM GPD does not implement it. Which editors do support this option?

The option for source tab as default is under jboss tools > web > editors which is
for the xml editors provided by the Web part of jboss tools.
I don't think jbpm even got access to these.

I have investigated further, and it seems that this is harder to reproduce than my
description suggests. As far as I can tell, I only get the Visual/Source tab when opening
a file 'for the first time' for one of my existing projects, which I have had for
a while. I'm not sure what the difference is, for example, but this might mean that I
am opening files ''for the first time since upgrading JBoss tools' or
something.
I guess this means two things: this report is less helpful than I hoped, since you
can't reproduce it, and that the priority is lower.
Should I investigate further, or would you prefer to close this as 'Cannot
Reproduce'? How does JBoss Tools determine whether the file has been opened before?

When file is opened and closed, then some setting saved in .metadata folder. You can
create new workspace, import you project into this workspace, in preferences set default
tab for vpe and open some pages in VPE to see on each tab they are opened.
