I've not yet been able to identify the exact sequence of events that creates the failure, but it happens when adding a field to an entity. There seems to be no noticeable pattern - sometimes it occurs when adding a single field, sometimes when running scripted adds of several fields. When it does happen, the only way to recover is stop Forge and restart it.
