The problem is that the method MenuManager.handleAboutToShow() at the end of menu creation
calls the MenuManager.update(boolean, boolean) method which merges the existing menu items
with the latest menu model.
At this moment the menu is full of items (because it's a menu created from a previous
step of the menu model creation). So, when an order of the menu model items differs from
what we have in the menu (this occurs in MultiPageEditor-s like ours) the
IContributionItem.fill(Menu parent, int index) method is used to put new actions from a
model into the menu. In case of item is ActionContributionItem the method fill() has the
following IF statement:

So, because the widget member is not null (at the moment of the menu is about to show) the
body of IF statement is never executed. For example, this happens with "Quick
Assist" which is contributed by AbstractTextEditor by its ID
(ITextEditorActionConstants.QUICK_ASSIST) as well as with other contributed actions.
In JSP/HTML Text Editors (StructuredTextEditor based, not MultiPage Editor) the count of
existing menu items and their order is completely equals to the new menu contents, so
ActionContributionItem.fill(Menu parent, int index) method is never invoked in this case
(the menu items just aren't changed at this step), so the menu is shown with all the
items.
It seems like a bug in ActionContributionItem.fill(...) methods.
There is a similar (but not the same) issue in BugZilla
(https://bugs.eclipse.org/bugs/show_bug.cgi?id=121529) on support bindings to existing
widgets in ActionContributionItem.

Thanks to Slava, I've made a patch that fixes the issue.
IMHO, the reason of the issue is either a bug in eclipse's pop-up menu extenders that
create menu item widgets by a repeatedly called MenuManager.update(boolean, boolean)
method or in MenuManager.update(bolean, boolean) method itself which cannot merge existing
(already created) menu items into resulting context menu.
Probably we have some extra menu items defined which cause this merge issue to happen with
the Manager.update(boolean, boolean) method.
I asked platform-ui-dev(a)eclipse.org regarding the issue, but was suggested to create an
issue in bugzilla as well as to provide an example on how to reproduce. I can't
quickly get a working example of a multy page editor with the same non-working menu
quickly (without using VPE's as a source editor and pushing too much code to bugzilla
as well)
Based on a suggestion that we can prevent an incorrect menu items merging by a cleaning
the menu items before a final call to MenuManager.upate(boolean, boolean) method, I
created a workaround patch that fixes the issue.

These changes are made to make Selection Provider relevant to MultiPageEditorPart which is
changed. This is more just to make it up-to-date and make to be instance of
MultiPageSelectionProvider, that to fix context menu issue. But I'm sure it's
important to have it up-to-date. (Some selection dependent actions enablement may work
incorrectly if we won't do this).
