The icon visible, but at another place. So user see to many buttons anywhere. From other
side we are going to hide most of them by default, possibly only refresh button will be
visible.So they mostly not used.
From other side almost all eclipse editors use main toolbar to use
icons, we should follow eclipse guidelines too.

#1 icon change/editor layout should be enabled only on visual/source tab
#2 icon refresh don't affect preview tab, but should.
#3 show folrmatting toolbar should be only enabled when visual part accessible.

Unfortunetly there no easy way to disable by default eclipse command toolbar contribution
item, so I have used workaround action sets mapping.
