OK so figured it out.. line 1177 of FileUpload.js says:
this.isFlash = swfobject.hasFlashPlayerVersion("9.0.28");
As my flash version was 9.0.124 I would get the empty add button.. once I moved to flash
10, everything was working as expected.
Seems like this is specific to flash/linux as I had tried it on a windows machine with
flash 9 and it appeared to degrade gracefully by creating a disabled add button.

I had noticed that in my application, if i have allowFlash="true" on my
rich:fileUpload that the "Add" button would suddenly turn into a white
rectangle.
I had also come across the same issue on the live site, so the best place to be able to
reproduce it would be there:
http://livedemo.exadel.com/richfaces-demo/richfaces/fileUpload.jsf?c=file...
Then make sure flash is enabled by checking the "Turn flash on" checkbox.
You will then notice the "Add" button has done from a plus symbol image and the
text "Add..." to simply a white rectangle
