I had also come across the same issue on the live site, so the best place to be able to reproduce it would be there: http://livedemo.exadel.com/richfaces-demo/richfaces/fileUpload.jsf?c=file... Then make sure flash is enabled by checking the "Turn flash on" checkbox.
