Eliminate a JBC Fqn level in clustered session caching

The Fqn we use has a level for the virtual host and another for the webapp. Combine into a single level with a ':' delimiting the two elements. Saves an unnecessary node traversal.
In the new combined element, put the webapp path first. For two different webapps, this is far more likely to different than the virtual host. So put it first so equals comparison will return false more promptly.

DistributedCacheManagerFactory becomes this:

The factory returns a different impl based on ReplicationGranularity.
FieldBasedDistributedCacheManager subinterface disappears.
Per-attribute operations throw UnsupportedOperationException if granularity is SESSION.
