It looks like this is due to a rule being fired at the wrong time
Here is the rule with details removed for clarity..ignore most of it except for model and
how it is retracted at the end.

I was able to resolve/workaround this issue by editing org.drools.util.BinaryHeapQueue to
make
dequeue(), dequeue(int) and enqueue() synchronized.
Don't know if there is a better way but I've been running my system for two days
without any problems.

I think the problem is due to a race condition in BinaryHeapQueue, the activation is being
fired twice.
The second time around the fact has already been retracted causing the exception.

Actually thinking about this some more, I'm wondering if the error is not thread
racing, but actually stale data. While we can only have one thread executing at the same
time each thread is responsible for the fact propagation and activation creation and
queueing. due to stale data in theory each thread could see out of sync versions of some
of the data structures. Hence while your synch on the queue methods worked. Synchronising
access to all data structures is not feasible, it would kill performance. I'm
wondering if there is actually more deep seated problems here. It seems like the only real
solution is the engine can only be executed in its Thread of creation, all other threads
would message their changes into that thread. Will discuss this with the other developers,
but its a 5.0.x thing, sorry.

Looking at the code some more.
You're correct that the inserts are protected via the lock, but the fireAllRules()
does not check that lock.
Therefore it is possible for a thread to call fireAllRules() and another to be calling
insert(), both end up modifying the same BinaryHeapQueue at the same time.
My workaround for this problem is to make the mutating methods of BinaryHeapQueue
synchronized. I have not had the problem since doing so.
This problem nearly cost me my job so if you push this to 5.x then I'll have no choice
but to use my private build with this fix in it.
