If you have hot deployment scanner disabled, then the application will not be undeployed when the ear is removed. Has the jboss-service.xml file under %JBOSS_HOME%/server/< serverName>/conf been changed by someone? By default the HDScanner MBean has ScanEnabled as true. Can you post the contents of that file?
