Have you looked for this pattern in your code?

  | try
  | {
  | 
  | }
  | catch (Exception e)
  | {
  |    transaction.rolllback();
  |    // no logs?
  | }
  | 

If you want to find the root cause of that exception, you should get some help from
someone on JCA.
