Progress so far:
I have created a simple test program which sends 20000 non persistent messages in a burst,
then consumes them all, and closes and creates a new consumer every 20 messages, with
prefetchSize set to 15.
With a small Thread.sleep as above this test locks up very quickly.
With the fixed code I have left it running overnight, and so far have have processed
around 25 million messages with no problems. I will leave it running further, but it looks
good so far.
Message delivery freezes with default values of prefetchSize and
slowConsumers

Message delivery seems to freeze under some circumstances with default values of
prefetchSize and slowConsumers.
Increasing the prefetchSize to 1000 seems to workaround the problem.
Waiting on customer to provide instructions for replication.
