ExtendedReceiver now extends ExtendedMembershipListener too. (These interfaces will all be merged into 1 in 3.0). #2 Make changes in FLUSH to (a) call unblock() on FLUSH completion if we use the push mode, or (b) add an UnblockEvent to the receiver queue if we use the pull mode
