@Darran - I'm not sure how FORM authentication relates to plaintext vs. digest
passwords. I also do not believe that the current approach works with mod_proxy and
Firefox.

Original code is:
String referrer = responseHeaders.getFirst("Referrer");
But the HTTP Referer header is actually spelled "Referer". Pull request with
