sorry, it does seem to be deployed by default (I missed it due to the fact that I was looking for taskform and not vacation2... I get the same error here. Most likely it is deployed wrongly since the taskforms are in a subdir. afaik, that should not be the case, so the taskform in the examples is (might be) currently 'wrong'.
