[jopr-dev] Don't see any template fields when adding a Queue

I'm trying to follow the recipe Ian describes in https://jira.jboss.org/jira/browse/JBAS-6219, but when I get to the "Add New" page I don't see any fields. Just hitting the save button fails when a queue with a null name/jndi-name is created.
